April 1, 2021

Dear SPS parent:

Free COVID-19 vaccines will soon be available to all SPS students who are 16 years old or older. Pfizer’s vaccine has been authorized for emergency use to vaccinate those aged 16 and up. If you would like your eligible child to receive the vaccine, please complete this SPS interest form by Wednesday, April 7.

Next Steps

·    In cooperation with Jordan Valley Community Health Center and Springfield-Greene County Health Department, SPS will offer vaccine clinics for eligible SPS students.

·    Once vaccine clinic dates and locations are established, required consent forms will be sent to parents/guardians who have completed the interest survey form. A completed vaccine consent form, signed by the parent/guardian, will be required for all students prior to being vaccinated.

·    Students 16 years or older are included in Phase 3 of the state’s vaccination plan, which opens April 9 and includes all Missouri residents.

·    As a reminder, to confirm your interest in a COVID-19 vaccination for your eligible child, please complete the SPS interest form by April 7.

·    If you have questions, you may contact SPS Health Services at (417) 523-1632.
